RISK MANAGEMENT FORMS
Risk Management Form A's must be completed - BEFORE the season starts - by all Coaches, Assistant Coaches and Team Mothers assigned to a team.
Place the completed form in an envelope with your name and team name written on the front of the envelope. PLEASE SEAL THE ENVELOPE and drop off at First Touch Soccer.
